Preparing for a Consultation
Thank
you for choosing naturopathic medicine.
Before you come for your appointment,
please fill out your patient
registration forms and send them
to me in advance. This allows me to
review your case and become familiar
with your history. It also eliminates
the need to ask you all the necessary
questions during the consultation, which
gives us more time to discuss your important
issues.
Filling
out the health history often gives patients
new awareness and an overview of their
condition in a way that can often be
missed in daily life. If you feel the
need to provide more information or
have questions, please feel free to
include these.
Please
be sure to read over all of the documents
included in the welcome packet and be
familiar with the office
policies.
If you
are currently taking any medications
(this includes prescriptions, birth
control pills, vitamins, supplements,
over-the-counter, etc.) please list
them on the appropriate intake form
provided. If you have a long list or
do not have the ability to write all
the medications, dosages, and frequencies
down, you may place the bottles in a
bag and bring them with you on your
first visit. I would be happy to assist
you in keeping track of your medications.
As a naturopathic
physician, I may ask questions that
seem unrelated to your current complaint.
I am often interested in details of
your condition that other healthcare
practitioners are not. The elaborate
details of where, when, how, and the
exact nature of the pain of a headache
is an example.
Often,
questions may be asked about how the
weather, environment, noise, heat, cold,
and many other conditions that may affect
you. Your likes, dislikes, and any personality
traits (the quirkier the better!) may
be requested. It is not required that
you answer every question posed to you,
however all information gathered about
you helps me to better understand you
and may help in your treatment. It is
often helpful if you consider your condition
in detail before your visit and detail
to me all the variables of which you
are aware.
You are invited to write down any questions you may have
and bring them to your visit. Often your questions can all be answered in the first office
consultation, however if you list is long or your questions require in-depth
conversations, we may need to focus on the most important questions and extend others out
over other visits.
